C. FIELD FATALITY LOG (SELECTED)
(Only a portion of the full casualty ledger. Names redacted where required. See Personnel Directory for full roster; access requires D-5 sign-off.)
F-01 — Agent Kelly Rhodes (Field Containment)
Assignment: Escort detail (Dorothy Marks) — Oct 31 deployment.
Last comms: “We’re near west perimeter. Visual—plume—” (Walkie log HX01-F-032, 23:49).
Status: Confirmed KIA (00:12, Oct 31). Cause: decapitation-consistent trauma. Body recovered west perimeter. Notes: Rhodes’ suppression harness recorded intermittent overload prior to severing event.
F-02 — Agent Thomas Pike (Field Containment)
Assignment: Bridge lockdown duty — Oct 31, 2014 (historical entry).
Last comms: “If anyone’s there—stay back.” (Scanner fragment HX01-HIS-2014)
Status: MISSING / Presumed Deceased (last sighting 31 Oct 2014). Coroner: unrecoverable remains. Logged as operational loss.
F-03 — Agent Michael “Mick” Callahan (Operations Lead)
Assignment: Perimeter leader — Oct 31 deployment.
Last comms: See bodycam transcript HX01-F-078 excerpt above.
Status: Confirmed KIA (00:54). Cause: decapitation-consistent trauma and secondary cauterization. Notes: call signs indicate entity targeted west perimeter despite monitoring lure elsewhere.
F-04 — Specialist ███ █████ (Technical Support)
Assignment: Audio traps / suppression arrays (HX-01B control node).
Last comms: “Array is spiking, full spectrum—shut it—” (telemetry log HX01-R-023).
Status: Confirmed KIA (01:22, Oct 30). Cause: blunt / laceration trauma consistent with secondary effects during breach attempt. Notes: Specialist was in proximity to electronics; corollary burns present.
F-05 — [REDACTED] (SHPD liaison)
Assignment: Night shift, Hollow’s Bridge precinct.
Last comms: Civilian channel; “they found them; it’s bad.” (public comms log)
Status: Confirmed deceased. Cause: decapitation signature. Notes: Local PD jurisdiction; D.A.P. forensics conducted joint autopsy.
Operational summary: Fatalities commonly present decapitation-consistent trauma with ritualized secondary modifications. Casualty pattern does not consistently coincide with D.A.P.-monitored bait sites; HX-01 has demonstrated capacity to strike away from engineered loci.

D. APPENDIX & CHAIN-OF-CUSTODY NOTES (ABRIDGED)
• All analog tapes digitized on intake; originals sealed under RR-34. Digital master copies archived in secure vault (HX-01 EVIDENCE VAULT). Access log requires dual-auth (Research Lead + Division Chief).
• Automated sanitizer applied to media with memetic triggers (name-choke events) per protocol MEM-02. Redacted segments are marked with timestamps and justification for redaction included in evidence packet HX01-RED/2017.
• Field audio with agent fatalities cross-referenced to coroner reports, sensor telemetry, and on-site camera footage where recoverable. Several camera feeds failed during high-static events; those failures are logged as CAM-FAIL with correlation to electromagnetic spikes.
Redaction key:
[REDACTED] — personal identifiers withheld.
[STATIC] — memetic / audio suppression artifact.
[UNINTELLIGIBLE] — low SNR; content unintelligible.
[FILE FRAGMENTATION / REDACTED] — data corruption / security redaction as per RR-34.
──────────────────────────────────────────────────────────────
NOTICE: These files contain memetic elements known to cause audio/name suppression and psychological effects on repeated playback. Access limited to personnel with memetic safety clearance (D.A.P. memetic certification required). Personnel must log all playback sessions and submit debrief reports to the Research Division (Dr. █████ Leroux) immediately following review.
